Yes it's like an appointment card with your next vaccine data on it and the information you mentioned so they know when and what you had.

There’s an app you check on , just place your thumb near the screen to register your chip

You should hopefully be able to ask your gp (and pay) for a letter stating you were vaccinated with X on these dates, in the future. That's not been confirmed yet though.
