I've had this debate many times and it really boils down to people that do and don't understand probability. The logic is the same whether its sexual health testing or "would you share your partner if you had one?" - firstly not everyone will lie - secondly, of those that do lie, not all can do it convincingly - finally, some of the people who can lie convincingly will later feel guilty and no-show. The bad news is that the people who slip through the cracks are probably psychopaths, but swings and roundabouts really.
