It's always the above cock length that's measured. I think it should be from - getting really specific here - the start of the protrusion from the abdomen, to the actual tip of the cock. I think it's measured as if using a wooden ruler, so if there's curvature unfortunately it may mean that the measurement's slightly less than if it was a flexible ruler that would touch the cock. That might mean losing a little bit of length, so is perhaps unfair.
