I tend to wriggle about until I get in a good position. It's more painful for me in all fours position. I either have to have my head right down with my shoulders or elbows on the bed or half kneeling with my hands holding the head board. If I'm not on a bed I improvise.

My size can be problematic in that position as well. It definitely varies from partner to partner, probably just the shape of the canal. Either way, I stay still with my back against a wall and I let the girl push her bum against me.
