I've had a few condoms come off in the past, but I very much doubt it was a size issue as I had them on nice and tight, I reckon it was probably a case of penetrating so deep that the base of the condom was entering inside the ladies, and gradually slipping off as I withdrew. These days, I check the condom every few minutes just to be sure its still on.

Happened once to me and in retrospect I think the condom was too small, can't remember the brand but it seemed smaller then the others I use and was a bit tight and didn't go all the way down easily

The lady got herself tested and all was good in the end
