I've seen pics of people wearing them and vaguely understand the principal but do they really work and are they not just uncomfortable? I can't say I've ever had an urge or need to wear one?

All I can say is try them, you might not like them, or you may love them, for me I'm a big fan, they also increase your girth quite considerably, not uncomfortable, just don't keep them on for hours, try the silicone ones first, they are easy to put on and take off and a lot cheaper than the metal ones.
