I'm not sure you quite mean 'against their will' - all kink generally needs consent of both partners, regardless of how much denial or frustration or desire to be released is involved.

Most people involved in chastity will have arrangements for cleaning, it will be different depending on each person's situation. For example, allowing removal for a few minutes each day for cleaning/hygeine.
