It’s a feedback loop.

The people on hot photos get hotlisted so people see their photos more. Ergo they get on hot pics more!

As many have said they are on there so get more traffic and thus more fabs. Its very hard for new people to appear there.

It's a lot better than it used to be, before the rule you could only have one pic in hot pics. The top ten would be mostly 2, sometimes 3 of the same people, just different pics.
