I'd imagine in most situations it would have to be at least brought up by the person who was going to be taking the cuckold role.

Most relationships will start off with a vanilla style level of sex. I think nowadays most people would feel safe suggesting some positions, additional toys, outfits or may be some light bondage (the type you find in Ann summers).

However, for more unusual kinks I would imagine that it comes best from the person who desires to be the person in lower power.

I.e. A girlfriend who suggests to her boyfriend that she humiliate him and sleep with other men is in most cases unlikely to get a successful outcome.

Similarly a woman or man who suggests that they would like to inflict pain on their partner may find their partner is no longer that into them.

However, it is far safer for it to be suggested the other way around.
