Depends in what context.

Sexually it will usually be the Dominant top with a sub bottom (straight,bi or gay)usually stated if other than straight.

Or it can simply be he's a good chap.

I thought it was a way of telling someone more than thanks, if that makes sense, ie someone that has gone out of their way to help someone etc, it seems to be used by men more than woman
