I've had the same just had to change condoms to a bigger size and it solved the issue, I think if it's not the right size it can restrict the blood flow

It's either your state of arousal or something to do with your blood flow.

Regarding you state of arousal how much porn do you watch? There are some good videos on youtube explaining how if you watch porn your brain is over stimulated with sexual images/thoughts. This means if you're with a lady (no matter how gorgous she is) you feel less aroused.

This would explain the symptoms you're experiencing. But as with anything symptoms can have lots of different causes.
