I'd say for the most part it just depends on the other guys boundaries. The only part of what you described (balls and cock near face whilst licking her out) might be an issue for some.

Are you concerned about cocks touching when she is sucking you both off? If so, I really would just try and go with the flow, enjoy the overall experience, fulfil her fantasy and you will feel far more comfortable next time.

Completely understandable. I don't really find a man 'attractive' in that way but I do admire a good cock and like doing stuff that makes everyone happy. I wouldn't consider myself fully bi as I am not equally happy with both sexes, but do consider myself bi curious and happy to display myself as such.
