Doggy style position, reach down the front, past your balls and reach that bit further. Your back will have to be fairly flexible, but I can manage to get the full fist in up to a couple of inches of wrist. I've done it in the past (vids floating about on the web) and found the angle the hardest part. Plenty of lube, warm up with toys, more lube and then find the best angle for you and then use more lube.

You may want to make sure you have plenty of lube. And short nails. And lube.
