FCS health form (travelling by ferry)

Hi

I have my ferry booked to sail to Bilbao on 19th July. I understand it is now compulsory to fill in a FCS Health form 48 hours before travelling to gain entry into Spain. I can only find a link to the form relating to air travel.

I have emailed Brittany Ferries but their operators are absolutely swamped just now and it could be sometime before I get a reply.

Anyone had any experience in obtaining the form?
