Can someone please tell me why I was asked to complete a security code before I could log on on my mobile. Its never happened before

Could someone have had access to your mobile phone without you knowing and tryed to log in ? If you put your password in wrong a few times your asked to enter a security code.

They also throw it in randomly too.
