keep losing fab shortcut icon

Anyone know why the fab shortcut icon on my phone (Android 4.4.2) disappears following a restart or power off/on cycle?

The site remains in my browser favorites, so it's only a few clicks required to put it back .... but no other icons are affected!

Any ideas? (phone software is up to date)
