They didn't they wore fabric or sheepskin hats possibly to keep their ears warm over the cap they wore the ritual bandanna.

In the photos I've seen of them they wear leather headgear, not helmets.

They would often not find a target and return to base. There were quite a few kamikaze pilots survived several missions.
