Usually if it's an ad on the TV it will say at the bottom in very small transparent letters (according to 100 people surveyed)

I'll quite often fill them in if requested to as I appreciate their value from a customer service perspective.

Best one I had to do was an alcohol consumption one (where I had to keep a diary of what I'd d*unk over a two week period) that coincided with my birthday, a weekend away and a generally pretty active social period - totting up my consumption at the end of it actually made for scary reading!!
