I think historically the majority of men who used them were not the type to out themselves and it's likely going to be the same in here. They were apparently more numerous in the UK but have become fewer in number now. The USA has generally a more relaxed attitude to sex venues, with far more venues that show porn, as an example - a lot of Europe is in front of us. So there are lots of public and private ones around elsewhere . We've also clamped down on public places people meet for sex, cruising, dogging etc - we're not particularly liberal.

Not all holes are fully anonymous, many would allow some visibility - sometimes even mirrors, so that you can see what's going on in the other side of the wall.
