I believe that a significant percentage of the population will have narcissistic traits, but far fewer have the formal diagnosis for which a number of traits would have to be identified together and is usually reserved for forensic (criminal justice) patients in this country.

It is thought that many sufferers avoid formal assessments, so there are perhaps more than statistics agree on, but still nowhere near as many as most guestimates or Internet scare stories

By the time someone has full blown NPD they are pretty delusional in my opinion. Some are overtly abusive but many are passive aggressive.
