NHS "Never Events"

Way over Yonder, that's where I'm bound

Horrible for the people that have experienced these but, shoot me now, 742 events across the whole of the NHS in four years sounds like it's likely to be quite a small percentage.

I haven't had anything go wrong with surgery on me but what horror stories do you all have?
