I always thought Cos and Romaine were the same. Cos being the English term and Romaine being the American one. And that Little Gem were the baby version of Cos/Romaine.

Small romaine heads best for it. Lettuce being the dominant igrediend, it has to be right. Good anchovies for the dressing and quality homemade croutons equally important
