Number 3 seems to be polarising.

The reason I included it in what I think everyone would have done is because from my person experience I've been having sex with someone before and someone else has briefly popped into my head. Weather that be a Porn star, celebrity, a previous lover or someone I'd love to have sex with. Not for the full duration but for a moment it has happened.

I define cheating as doing something with someone / in connection with someone else that you wouldn't tell your other half about. (Though it doesn't include the scenario where you admit to having sex etc with someone else without your OH's consent.)

The only time I think about someone other than the person I'm fucking during is when I make eye contact with the other person I think.

Exactly this. Informed and enthusiastic consent is such a fantastic way of putting it.

I think if you're lying, hiding things... it's cheating. You don't have to say every little thing you do, every time you send someone else a suggestive message for example. But if you never mentioned it, told the other person you weren't doing that etc... then yes you are.

I don't think cheating is black and white, in to neat little categories. It's so nuanced and depends on the relationship you have with another, what's been discussed.
