None of us know the ops circumstances or why he finds himself in this situation. We do know that his life lacks love and affection. We also know that in general men's mental health is at an all time low. In my opinion constructive advice is more compassionate than criticism.

Op I'm not sure if I've mentioned it before but have you considered couples counselling, I'm pretty sure you can go alone. It might help you and in the long run your wife and relationship

I think you need to leave your wife and start afresh, you've been here a while now and as somebody who was in a similar situation although never cheated on my wife, it was difficult my daughters who I was close to don't talk to me but I had to leave for my sanity, my ex now talks to me which is great she didn't and doesn't understand why I left but we are friends. It's only you that can make the decision, good luck OP.
