Like an mot all that means is that the person didn’t have that infection at time of testing.

Even prep has potential health impacts

I’ve nursed people with aids and hepatitis it ain’t pretty!

Some STI’s can’t be tested for on men ie HPV it can be passed between men and then passed onto a female partner (even with a condom) where it will show up in tests and can be linked to cervical cancer.
