From my point of view, yes. Always! Otherwise it's a potential distraction from the intimacy you are sharing. And occasionally, there are some who wilt at the idea of, or struggle with, putting one on (that's when you need to apply by mouth). Oh yes; hands and/or mouth. Less frequently with my mouth though for a couple of reasons: my last few partners have been quite girthy which is a problem in itself plus I was a late bloomer with wisdom teeth and can't always get the nack now.
