How does something like this happen? Especially after Brandon Lee.

Someone has the role in f prepping the weapons on the set before the actor is given it. So I guess it will be that person. Unless Baldwin swapped out the prop for a real gun

The brandon lee incident wasn't even a bullet that killed him, it was some debris in the barrel propelled out by the blank round.

Maybe they need to stop using actual guns that have been fired. Surely with todays tech they can create a gun that doesn't need blanks but still looks legit. Afterall, isnt most of the sound added post filming?
