What makes it extreme is when you know its too much for you. Its very much dependent on your own desires and limits.

I've played in situations where the deepthroating technique has been used to induce a (very) mild sense of Hypoxyphilia, or where the gagging sensation has been a sexual stimulus in itself.

Yes i think your right .. you would really have to trust the person doing this .... I would worried i would be sick all over the place .. as know i can do it but know my limits .. if to extreme.
