Iknow the answer to this, i know its silly thread day but the real answer is they give them pictures to look at like they do with children.

Another sensible (but pointless) answer.

In rural China, where many are illiterate, they use a symbol a bit like a capital E (it's the character for mountain when laid on it's back). The testee has to indicate whether the character is pointing up, down, left or right.

I knew that bit of info would come in useful one day...
