I wouldn't comment to anyone about their pictures because it's none of my business.

Likewise I can change my profile picture to me shoving a large carrot up my arse if I want and it's nobodies business (I won't)

If they asked I would say

I think a pic of x (insert body part or description ) would be a really good way of showing your Best attributes

So you’re focusing on positives rather than the negative of the short comings of the current profile pic
