I had a friend of mine sectioned in a secure health facility because his behaviour was becoming increasingly abusive erratic and bizarre; he was detained, medicated and he recovered.

Should I come clean to him and let him know why he was detained; he is clearly unwell again, has been threatening, abusive and paranoid, is unwashed unkempt and uninhibited in public.

No if you think he's unwell again you know what to do.
