Yes, it can be rather uncomfortable but it depends how sensitive your clit is, what you're wearing underneath and what you're doing.

I would have given some doubt towards her unawareness, but I genuinely haven't met a single woman who wouldn't check how she looks in her clothes before going out. And the gyms are usually full of mirrors too...
