Does anyone swear they've left a note on someone's profile but a few days later it's gone?

Yes 100%. They are there on the profiles, we view them as a reminder but when visiting again they are no longer there.

Do they require saving after every view? X

No, just after adding - editing.

Private notes, not profiles.

If the profile went UNLOS then the OP wouldn't be find a profile to notice the note he made on it was missing....as the whole profile would be missing.
