Blocklist fail!

I keep coming across profiles I have blocked which have somehow become "unblocked". The profiles are not new and I make notes and put the date when I block and these notes are still there!? I know I havent unblocked them so how is this happening?

Best bet is to contact admin with some details - examples of profiles in question. Not something I've come across before myself.

This has happened to me too. I realised I was still friends with them? Blocked again and I don't think it's happend since. Does that or should that make a difference?
