How can it show that a user has accepted my friends request then we are friends when I have never sent request.

Speak to admin to check your account hasn't been hacked, not normal, but they will be able to help and reset if needed

I have done this so many times. Meant to add profiles to our hotlist but prodded the friend invite instead

I have noticed this several times myself, there was one today in my friends updates. I never invite guys as friends so this must have been a mistake. I'll report it to admin next time.
