how can someone be photo verified and have fake photos up its putting users at risk

We've encountered plenty of photo verified fake people. The admins clearly don't pay much attention and look out of signs of photo editing or even do a simple reverse image lookup on yandex.

so the first thing we do to anyone now, is lookup their images. Can't trust anyone eh?

The Admins are very good at spotting fake signs and pics. the PV pics are then checked by a second Mod

If you think someone is fake report the profile and it will be looked at
