We went to Le Mask. Smallish, classy cocktail bar feel. Was very nice, sexy and fun.

les Chandelles for the uber hot people. Very strict door policy, we didn't go, but is supposed to be good IF you get in.

Yes, can't remember exactly, but near 2 or 3 metros in central Paris. We were staying near Notre Dame and got a taxi for 10-15 €. They have a website maybe in French though.
